What Is Airway Orthodontics?

Airway orthodontics is an approach to orthodontic evaluation and treatment that considers more than tooth alignment alone. This can involve looking at the width and development of the upper jaw, the way the teeth fit together, available space for permanent teeth, and other structural features that can influence treatment planning.

At Heiner Orthodontics, this approach is especially relevant for growing children, whose jaws and dental arches are still developing. Depending on the patient, treatment may include expansion or other orthodontic appliances designed to address narrow arches, crowding, or bite concerns.

Airway orthodontics does not, however, replace medical evaluation for breathing or sleep disorders.

Benefits of Airway Orthodontics

An evaluation around ages 7 to 9 can create opportunities to address structural orthodontic concerns while a child is still growing and support a broader, well-aligned smile, which in turn may help with emerging breathing-related problems.

Supports Healthy Arch Development

A narrow upper arch can leave limited room for permanent teeth and contribute to crowding or bite concerns. When expansion is appropriate, orthodontic treatment can help create additional width in the developing upper jaw. Dr. Heiner has used expansion-focused treatment for decades and evaluates each patient individually before recommending this approach.

Creates Space for Developing Teeth

Airway-focused treatment may help create room for permanent teeth as they emerge. By addressing certain width or spacing concerns during growth, treatment can guide dental development and potentially reduce the severity of crowding later. The specific approach depends on the child’s age, dental development, bite, and individual orthodontic needs.

Addresses Bite and Jaw Relationships

The width and position of the upper and lower arches influence how the teeth come together. An airway-focused orthodontic evaluation allows Dr. Heiner to consider these relationships when planning treatment. Depending on the patient, treatment may address concerns such as narrow arches, crossbites, crowding, or other developmental bite issues.

Takes a Broader View of the Developing Smile

Dr. Heiner’s treatment philosophy places particular emphasis on smile width and creating adequate room for the teeth. Rather than looking at individual teeth in isolation, he considers the overall shape and development of the dental arches when planning treatment, helping create a balanced foundation for the patient’s future smile.

Who May Benefit from Airway Orthodontics?

Children with signs of limited upper-jaw development or other orthodontic growth concerns may benefit from an airway-focused orthodontic evaluation. At Heiner Orthodontics, Dr. Heiner particularly encourages early evaluations for children around ages 7 to 9, when growth and dental development are still happening.

During an exam, he may look for concerns such as:

  • A narrow upper dental arch
  • Crowded or emerging permanent teeth with limited space
  • Crossbites or other bite discrepancies
  • Jaw or arch development that may benefit from expansion
  • A smile that appears narrow because of limited arch width

These findings do not necessarily mean treatment is required. Rather, they help Dr. Heiner determine whether monitoring, orthodontic treatment, or evaluation by another healthcare professional may be appropriate.

How We Provide Airway Orthodontic Care

Airway-focused orthodontic care can look different from one patient to another. Dr. Heiner chooses treatment based on the patient’s growth, arch development, bite, and orthodontic needs, using a combination of digital evaluation and customized appliances when appropriate.

Early Orthodontic Evaluation

Early evaluations between ages 7 and 9 allow Dr. Heiner to see how a child’s jaws, arches, and permanent teeth are developing before all adult teeth have erupted. Some children will benefit from active treatment, while others simply need continued monitoring.

Rapid Palatal Expansion

A rapid palatal expander is an orthodontic appliance used to widen a narrow upper arch, which can create additional room for developing teeth and help address certain bite relationships. Dr. Heiner has incorporated palatal expansion into his orthodontic approach for decades and determines timing based on each child’s growth and anatomy.

Invisalign Palatal Expander

For qualifying younger patients, the Invisalign Palatal Expander offers another way to support expansion of the upper arch. Rather than relying on a traditional fixed expander design, this system uses a series of digitally planned devices. Dr. Heiner will evaluate whether this option or a traditional expander is better suited to each child’s needs.

Invisalign® and Braces

After expansion or as part of a broader treatment plan, Invisalign or braces may be used to guide the teeth into improved positions and refine the bite. Heiner Orthodontics offers multiple orthodontic options, allowing Dr. Heiner to choose the method that best matches the patient’s age, treatment goals, and complexity.

Temporary Anchorage Devices (TADs)

Temporary anchorage devices, or TADs, are small orthodontic anchors that can provide additional control for certain tooth movements. Dr. Heiner may incorporate TADs into more complex treatment plans when additional anchorage is beneficial.

Coordinating With Other Healthcare Providers

When a patient’s symptoms fall outside the orthodontic scope, additional assessment may be appropriate from providers such as an ear, nose, and throat physician, myofunctional therapist, primary-care provider, dentist, or sleep specialist. Heiner Orthodontics can incorporate relevant findings from other providers into orthodontic planning when appropriate.

Airway Orthodontics Treatment Process

While evaluation and treatment are always unique to each patient’s needs, this is a general overview of what you might expect throughout the process:

1. Complimentary Orthodontic Consultation

During the initial consultation, Dr. Heiner evaluates the teeth, bite, arch width, and stage of dental development while discussing concerns with the patient or parent. This visit helps determine whether treatment, continued observation, or additional evaluation may be appropriate.

2. Digital Imaging and Treatment Planning

When additional information is needed, Heiner Orthodontics may use digital scans and 3D CBCT imaging to better understand the patient’s orthodontic anatomy. Dr. Heiner uses these records to develop a customized treatment plan based on the position of the teeth, arch relationships, growth, and other relevant orthodontic findings.

3. Expansion or Orthodontic Treatment

Depending on the diagnosis, treatment may involve a traditional palatal expander, Invisalign Palatal Expander, Invisalign clear aligners, braces, TADs, or a combination of approaches. Not every patient needs expansion. Our recommended treatment is based on what Dr. Heiner believes is appropriate for the patient’s specific orthodontic needs and stage of development.

4. Progress Monitoring

Regular orthodontic appointments allow Dr. Heiner to monitor how the teeth, bite, and dental arches are responding to treatment. Adjustments may be made when needed. If concerns arise that require medical evaluation beyond orthodontics, families may also be encouraged to consult an appropriate healthcare provider.

5. Finishing and Retention

After the active phase of orthodontic treatment, the teeth are refined toward their planned positions and retainers may be used to help preserve the result. Dr. Heiner provides personalized retention instructions based on the type of treatment completed and continues to monitor the stability of the patient’s smile as appropriate.

Cost of Airway Treatment

The cost of airway-focused orthodontic treatment varies because each patient may need a different type or combination of care. An early evaluation may lead to monitoring alone, while another patient may benefit from active treatment.

Heiner Orthodontics will review recommended treatment and associated costs before care begins. While we are not in-network with carriers, we accept all insurances and can help families understand applicable orthodontic benefits. Flexible financial arrangements are also available, including in-house payment options, outside financing through CareCredit, and family discounts.

Our team wants families to understand both their clinical and financial options before making a decision.

Airway Orthodontics FAQs

  1. No. Heiner Orthodontics does not diagnose sleep apnea or other medical sleep and breathing disorders. Dr. Heiner evaluates orthodontic factors such as jaw development, arch width, tooth alignment, and bite relationships. If symptoms suggest that medical evaluation is needed, a patient may be encouraged to see an appropriate healthcare provider.

  2. Dr. Heiner encourages parents to consider an early orthodontic evaluation around ages 7 to 9, particularly when there are concerns about dental development, crowding, narrow arches, or bite relationships. An early visit does not automatically mean treatment will begin. Sometimes the best recommendation is simply to monitor growth over time.

  3. Dr. Heiner evaluates orthodontic features such as the width of the upper arch, available space for permanent teeth, crowding, bite relationships, and jaw development. Digital scans or 3D imaging may also be used when appropriate. These findings help guide orthodontic treatment decisions but are not used to diagnose medical breathing or sleep conditions.

  4. Yes, when a patient has an orthodontically narrow upper arch and expansion is appropriate. Palatal expansion can increase the width of the developing upper arch and create more room for teeth. Whether expansion is recommended depends on the patient’s age, growth, bite, anatomy, and overall orthodontic treatment needs.

  5. Depending on a patient’s needs, Dr. Heiner may recommend a traditional palatal expander, Invisalign Palatal Expander, Invisalign treatment, braces, TADs, or a combination of orthodontic approaches. Treatment is selected based on the individual patient’s growth, dental development, bite, and treatment goals rather than following one standard airway protocol.
